Referrals may be initiated by:

• Family Members or Legal Representatives
• Friends or Community Advocates
• Self Referrals
Admissions Process
Step 1: Referral Submission
The referral process begins with completion of our Online Referral Packet.
The packet helps us understand:
• Medical and cognitive support needs
• Safety considerations
• Current living situation
• TennChoices eligibility
• Individual preferences and goals
Step 2: Application Review
All completed referral packets are reviewed by the Board of Directors.
The review focuses on:
• Eligibility and alignment with TennChoices Programs
• Individual needs and required supports
• Safety considerations for the client and household
• Overall fit with the home environment and care model
Step 3: Approval & Orientation
If approved, families and referral partners will participate in an orientation process, which includes:
• Placement timelines
• Transition planning
• Overview of services and supports
• Expectations for residents, families, and providers

Who determines eligibility for placement? Eligibility and placement decisions are reviewed by the Board of Directors based on referral information, safety considerations, and program alignment.
Does a medical diagnosis automatically qualify someone for placement? No. Each referral is reviewed individually to ensure the home environment can safely and appropriately meet the person’s needs.
Are these homes institutional or clinical settings? No. Our homes are community based residential settings designed to support individuals in their Least Restrictive Environment, even when medical complexity is present.
Can families remain involved after placement? Yes. Family involvement is encouraged and supported, consistent with the individual’s preferences and rights.
Is there a waiting list? Placement availability varies. Approved applicants will be informed of anticipated timelines during the orientation process.
