About Us
PB&J LRE Housing Solutions Inc. is a nonprofit organization dedicated to developing safe, affordable housing for individuals with complex medical and cognitive needs – so they can live with dignity in their Least Restrictive Environment (LRE). 
We support family-owned care homes and shared service models that strengthen quality, consistency, and safety while reducing unnecessary institutional placement. Our work is rooted in a simple belief: medical complexity should never eliminate the right to a home-like, community-based life.
Our Mission
Our mission is to provide affordable housing that protects the most vulnerable citizens who qualify for TennChoices Programs, ensuring they can live with dignity, safety, and appropriate support in community based settings.

Our Story
Our organization was born from lived experience – not a business plan.
The birth of our first child changed our lives forever. Although he appeared healthy at birth, we later learned he had a genetic mitochondrial disorder, which evolved into significant intellectual disabilities, autism, and complex medical fragility.
Daily life required advanced medical knowledge, constant vigilance, and deep emotional resilience. As our son grew older, one question became unavoidable:
What happens when we can no longer provide his direct care?
We searched for a true “forever home” – one close to family and friends in Shelby and Fayette Counties – that could meet both his medical and cognitive needs without forcing him into an institutional or overly restrictive setting.
That search revealed a painful truth: the system was not built for individuals like our son.
PB&J LRE Housing Solutions Inc. was created to change that.

Our Model 
Housing with Support Ecosystem 
At PB&J LRE Housing Solutions Inc., our model is built around housing paired with a comprehensive support ecosystem—designed to enhance quality of life, safety, and inclusion for individuals with complex medical and cognitive needs.
Our approach ensures individuals can remain in their Least Restrictive Environment (LRE) without being forced into institutional settings simply because their needs exceed the limits of traditional group homes
Bridging the “Acuity Gap”
Many residential options serve individuals who need help with daily living but are medically stable. However, many adults with developmental disabilities also experience high medical acuity, including:
         • Seizure disorders
         • Feeding tubes
         • Respiratory dependencies
         • Complex medication regimens
Our model develops specialized, community based homes that integrate skilled nursing oversight into residential settings—preventing unnecessary placement in hospitals or nursing facilities.
This approach demonstrates that high acuity care can be delivered in a home like environment, not just in clinical institutions.
Person Centered “Aging in Place”
Frequent transitions—from home to hospitals or large institutions—can be deeply traumatic, especially for individuals with developmental disabilities and autism.
Our homes are designed to support aging in place, allowing residents to remain in familiar surroundings as their needs evolve, preserving stability, dignity, and emotional well being.
Homes may include:
      • Built in Medical Infrastructure
      • Ceiling lifts and accessible layouts
     • Oxygen storage and medical equipment accommodations
     • Specialized sensory rooms

 

Addressing the Limitations of Current Medicaid Models
Traditional Medicaid funded housing often struggles with:
       • Fragmented funding for medical vs. behavioral care
       • Limited clinical training for direct care staff
       • Residential layouts not suited for medical complexity
Our model addresses these gaps by providing:
       • Specialized home designs for medically complex residents
       • Private bedrooms for dignity, privacy, and individualized care
       • Integrated environments that support medical oversight and cognitive support together
Residents are never forced to choose between behavioral support and medical care—they receive both safely and appropriately.

As a nonprofit organization, PB&J LRE Housing Solutions Inc. provides shared services coordination to support family owned care homes and partner providers. 
Shared services may include:

       • Property security and maintenance coordination
By centralizing these services, care homes can focus on what matters most: delivering high-quality, person-centered care.
